Description

Clinical Professionals will provide telephonic care navigation and education to patients with MSK issues impacting the Shoulder, Hips, Knees, Cervical, and Lumbar Spine.

Clinical professionals will gather pertinent medical history as appropriate for assessment of the current treatment algorithm and the potential for additional treatment options or formal medical opinion.

Responsibilities include:

Triage (initial) and follow up (subsequent) on patient's symptoms, treatment, and care satisfaction

Medical/surgical decision support

Contact and advocate on the patient's behalf with the current treating physician if a clinically concerning situation identified

Connection and engagement with Virtual physical therapy if warranted

Connection and scheduling with Local Physical Therapy clinic if warranted

Connection and scheduling with a high-quality MD Expert for diagnosis consideration and treatment plan formation.

Connection and scheduling with a local high-quality surgeon for Consultation.

Connection and engagement with a virtual expert second opinion

Connection with a digital home engagement platform for chronic pain, stress management, posture, ergonomics, and home exercise program.

Member education

Education on the benefits of a surgical second opinion when elective surgery is being considered.

Verbal education and delivery of educational materials digitally

Clinical Professionals will review care history with patients with care navigation and educational focus to reinforce current treatment instructions and/or education on additional treatment options.

Clinical Professionals will exercise clinical judgment in life and limb-threatening situations for proper connection and communication to the current treating provider or emergency medical services if warranted.

·         Patient-reported signs of Neurosurgical Emergency (spine)

·         Patient-reported DVT signs post-surgically (Hip and Knee TJR)

·         Prescription medication misuse

Minimum Requirements:

·         Licensed Practical Nurse with 2 -3 years of healthcare experience.

·         Orthopedic or Trauma surgery background preferred

·         Excellent verbal and written communication skills, Bilingual a plus

·         Must have a clear and active license to practice in the State of Residence

·         Knowledge and experience with motivational interviewing a plus

 

Salary commensurate with experience.  Full-time positions are eligible to participate in company benefits including Medical, Dental, Vision, Life Insurance, LTD, AD&D, Spouse, and Dependent Life.  Retirement plan (401k) with company match, paid holidays, and paid time off.

Flexible Schedules are available full and part-time, Monday – Friday between 8 am and 7 pm On-site in the Tampa office.  May consider a hybrid or remote opportunities.
