From admission to discharge, you'll assess needs, coordinate services, advocate for patients and families, and ensure every individual receives a smooth, well‑planned transition to the next level of care.
In this role, you'll use your clinical expertise and critical thinking skills to support patient safety, experience, and quality outcomes. You'll help manage length‑of‑stay goals, implement best practices, and contribute to a care management model that consistently delivers excellence.

Required Education:
• Graduate of an accredited School of Nursing, required
• Bachelor's Degree of Science in Nursing, minimum required
Experience:
• 2-3 years acute care hospital setting, preferred.
• Care Management or Utilization Management experience, preferred
Licensure:
• Current License in the state of employment, required
Certifications:
• BLS (CPR) at hire date, required, or within 90 days of hire
• Certification in Case Management, Preferred
