The patient navigator reports to Orthopedics Practice Manager and is responsible for coordinating the care for patients of the Heartburn Center. This position will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. The patient navigator performs duties and responsibilities in a manner consistent with our mission, values and Mercy Service Standards.


  • Experience:Minimum of 3 years nursing experience 
  • Required Education:RN; BSN preferred 
  • Licensure:Current Registered Nurse 
  • Certifications:Current BCLS certification
  • Other:Ability to read and communicate effectively in English. Good computer skills. Team player with excellent communication and interpersonal skills with all levels of internal and external customers (patients, medical staff, support/technical staff, outside agencies, community). Additinal langusges preferred.
