General Summary of Position
Provides professional nursing care for a disease specific subset of stroke patients as part of an essential component of Stroke Services. Navigation services are expected to provide individualized assistance to patients upon entering and being discharged from the health care system who have experienced an ischemic or hemorrhagic stroke. Serves as a resource for stroke patients and their families, physicians, and others serving in the role of educator, quality improvement champion, and consultant to patients and other key stakeholders. This is done in collaboration with the divisions of Neurology, Medicine, and Neurosurgery. These functions are performed in accordance with all applicable laws and regulations and Medstar Health's along with the hospital entity's philosophy, policies, procedures and standards.


Primary Duties and Responsibilities

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Develops, plans, and implements evidence-based measures to increase the quality of stroke care as evidence by achieving and maintaining appropriate Joint Commission Stroke Certification and MIEMSS designation.
  • Performs professional clinical service in the inpatient and outpatient clinical setting including, but not limited to: patient assessment, communication between patient and provider, communication between MFSMC and referring physicians, identifying and decreasing barriers to care.
  • Collaborates with the Stroke Coordinator to ensure compliance with guidelines.
  • Assists in collecting and reporting stroke quality data measures.
  • Works in collaboration with the Stroke Coordinator to develop action plans to address opportunities for performance improvement related to care of stroke patients.
  • Collaborates and develops relationships with physicians and other departments to facilitate and support quality stroke care.
  • Coordinates patient education; serves as a resource for patient and families during their treatment period; collaborates with other care providers to meet the needs of the patient and family during inpatient and outpatient phase; assesses patient's learning style and health literacy in order to provide education on the appropriate patient level.
  • Collaborates with community organizations and resources to provide education and events related to stroke prevention and care.
  • Communicates with patient and/or family 24 to 48 hours after discharge and up to 90 days after discharge to ensure patient understand importance of discharge care and follow-up.
  • Identifies opportunities for patient participation in clinical trials whenever feasible. Communicates information to research coordinators and referring physicians as necessary to promote enrollment.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
  • Participates in multi-disciplinary quality and service improvement teams.
  • Performs other duties as assigned that are appropriate to the position.

    Minimum Qualifications
    Education

    • BSN required

    Experience

    • 3-4 years Professional clinical experience required and
    • 1-2 years 2 years of recent experience in neurological care preferred and

    Licenses and Certifications

    • Current RN License required and
    • Basic Cardiac Life Support, Advanced Cardiac Life Support required and
    • CNRN, SCRN or certification by a comparable board preferred

    Knowledge, Skills, and Abilities

    • Knowledge of professional nursing theory and practice to assess, plan, implement, and evaluate patient care.
    • Knowledge of medical equipment and instruments to administer patient care; Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care; Skill in establishing and maintaining effective working relationships with patients, medical staff, and the public.
    • Ability to react calmly and effectively in emergency situations.


    This position has a hiring range of $87,318 - $157,289
