Description

Responsible for the development and oversight of community and population health services as part of Care Transformation department for the Baltimore Region hospitals - MHH, MGSH, MUMH, and MFSMC. Position is key to the regional care transformation team and oversees external outreach divisions of each hospital, community health nursing, community benefit reporting, and community health needs assessments. Supports the strategy, design, implementation, and evaluation of the hospital's population health activities. Responsible for identifying opportunities for quality improvement for service delivery and outcomes, ensuring compliance with State and Federal regulations.
Education

  • Master's degree in public health, social work, public administration, business required or
  • related fields required or
  • equivalent experience. required
Experience
  • 5-7 years Progressively more responsible job-related experience. "thorough knowledge of maryland and federal health policyprograms impacting community health." Experience with or understanding of clinical considerations, and social determinants of health. Experience with computer systems required, including web-based applications and microsoft office applications which may include outlook, word, excel, powerpoint or access. required and
  • Managerial experience; knowledge of cerner emr and crisp database preferred
Licenses and Certifications
  • Valid RN license in the State of Maryland required

  • Develops department goals, objectives, policies, and procedures. Ensures compliance with business unit policies and procedures and governmental and accreditation regulations.
  • Develops and recommends department operating budget and manages resources according to approved budget.
  • Selects, trains, orients, and assigns department staff. Develops standards of performance, evaluates performance, and conducts performance management planning. Initiates or makes recommendations for personnel actions. Maintains ongoing communication with subordinates to review programs, provide feedback, discuss new developments, and exchange information.
  • Leads Community Health Needs Assessment in conjunction with Corporate Community and establishes implementation pathways for the alignment, integration and development of new community health programs.
  • Leads regional equity strategy in partnership with system department of human resources and quality/safety.
  • Executive oversight to reporting compliance with agencies with interest of population and community health programming, processes and outcomes (i.e. HSCRC, IRS).
  • Monitors existing and forms new relationships with community leaders, organizations, state and local agencies to form coalitions and programs that address the social determinants of health in populations that the hospitals serve.
  • Serves as departmental liaison to other corporate shared services (i.e. Clinical Care Transformation, Corporate Community Health, MedStar Family Choice, VNA etc.) within the operations function, including system-wide committees and state-sponsored workgroups.
  • Works with executive leadership to develop strategy and initiatives related to community health programs including implementation schedules, resource estimates, financial impact and ROI analysis, where appropriate.
  • Provides general administrative direction and support services including, but not limited to community health nurses, health educators/coordinators, grant specialists, and CB analysts.
  • Provides oversight and guidance around departmental functions such as: operational planning, vendor relationship management, budgeting, and financial controls as well as workforce management, regulatory compliance and communications.
  • Requires skills in leading group meetings, communicating findings (oral and written), interviewing, analyzing, negotiating, contracting, and writing action plans and performance contracts with staff. Must have the ability to network and make connections with community health leaders.
  • Participates in meetings and on committees and represents the department and business unit in community outreach efforts.
  • Participates in multi-disciplinary quality and service improvement teams.
  • Performs other duties as assigned.
