General Summary of Position
Supports home health and home infusion services. Builds and maintains positive customer relations with referral sources and key stakeholders at the MedStar hospitals. Generates accurate referrals utilizing the most efficient processes and tools available in MedStar. Contributes to creating an environment of teamwork; develops trust with co-workers and with other Agency staff.


Primary Duties and Responsibilities

  • Contributes to the achievement of established goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Completes annual Code of Conduct, Compliance, and other mandatory training. Complies with governmental and accreditation regulations.
  • Care Coordination: Collaborates with the hospital clinical team to develop and process home health referrals. This includes, but is not limited to, offering patient choice, educating patients/families on what to expect from home health services, and arranging these services.
  • Customer Service: Seeks to build new and maintains effective working relationships with referral sources, key stakeholders, and other departments. Participates in meetings and on committees supporting the Department and Agency marketing and outreach efforts.
  • Resource: Serves as a resource to referral sources including case managers, physicians, and home care operations on home health services and practices as well as the home health referral process. Provides education to referral sources, providers, members of the clinical team regarding home health and infusion services.
  • Processes on average a minimum of 80 referrals per month.
  • Professional Development: Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.
  • Teamwork: Collaborates with the Home Care Coordination Team in the daily routines of referral development and home care coordination including monitoring daily census, keeping detailed daily logs/statistics, and keeping workload organized and prioritized. Consults with and supports referral sources and home health and home infusion agency staff on complex cases.
  • Technology/Workflow: Utilizes technology to effectively maximize efficiency and improve accuracy and communication of daily care coordination activities. Documents care coordination activities in hospital case management and MedStar My Health plan electronic database. Ensures appropriate information is included in the patient's medical record. Is proactive in developing workflow practices that support Agency needs.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams.

    Minimum Qualifications
    Education

    • Associate's degree or an equivalent combination of education and experience required
    • One year of relevant education may be substituted for one year of required work experience.

    Experience

    • 1-2 years Related experience in a healthcare setting required
    • One year of relevant professional-level work experience may be substituted for one year of required education.

    Licenses and Certifications

    • Upon Hire required

    • Strong interpersonal skills, including verbal and written communication.

    This position has a hiring range of $18.33 - $31.61
