General Summary of Position
Functions as a core member of a collaborative care team that involves the patient's primary care provider psychiatric consultant and other mental health providers. The Care Manager is responsible for coordinating and supporting mental health care within the care setting and for coordinating referrals to clinically indicated services outside the care site.
Primary Duties and Responsibilities
Education
- Master's degree in Psychology Social Work or related field required
- BSN or LPN with mental health training/certification may be substituted required
- 1-2 years Related experience in a similar care model preferred
- Experiencing working with patients who have co-occurring mental health substance abuse and physical health problems. Must have experience with screening for common mental health and/or substance use disorders patient assessments and treatment planning required
- LCSW-C (Licensed Certified Social Worker-Clinical) LMSW (Licensed Masters Social Work) or LCPC (Licensed Clinical Professional Counselor) required
- BSN or LPN with mental health training/certification accepted required
- PhD in Psychology may be substituted required
- Strong communication skill.
- Demonstrated ability to collaborate effectively in team setting.
- Ability to effectively engage patients in a therapeutic relationship.
- Working knowledge of differential diagnosis of common mental health and/or substance use disorders.
- Knowledge of psychopharmacology for common mental health disorders.
- Basic computer skills preferred.

USD $61,838.00 - USD $111,259.00 /Yr.
