The compensation range noted above represents the base salaries for all positions at a given grade across the health system. The CHW provides chronic diseasea education, smoking cessation counseling and links patients to mental health resources and other social determinants of health. We are seeking a Community Health Worker who, under the general direction of the Senior Community Navigator and Manager, collaboratively provides comprehensive care management services in response to e-referrals, and assists patients/clients to navigate and access community services that will have a positive impact on their overall health. This position requires a non-smoker for at least 2 years . REQUIREMENTS: High School Diploma or equivalent; bi-lingual or multi-lingual preferred; driver#s license and vehicle; must complete smoking counselor training within one year of hire; completion of#CHW Core Competency Education in Massachusetts; trained and certified in chronic disease management such as asthma, hypertension, diabetes and mental health; pediatric experience a plus Our competitive salary package is adjusted to align with a candidate#s relevant experience and skills.
