Job Summary:
The Quality Coordinator is responsible for assessing and meeting the needs of patients and families. The Quality Coordinator supports the Care Management team, ensuring all pertinent patient information is shared amongst team members. They regularly assess the practice for compliance within various programs and offers suggestions to improve workflows. The Quality Coordinators makes certain the patients gaps in care are being appropriately addressed.

Job Essentials: The ideal candidate will meet the following requirements and competencies.
- Prior experience monitoring the quality of patient care preferred
Education Requirements:
- HS Diploma or GED plus technical schooling
License/Certification/Registry:
- Medical Assistant Certification required; LPN License will be considered

The rate of pay for each position will be determined based on a variety of factors including the candidate's relevant experience, qualifications, skills, etc. The minimum rate does not include bonuses/incentives, differential pay or other forms of compensation.
