Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Functional Statement #:Licensed Practical Nurse, T38 Hybrid - GS-0620-03T6 Appendix G13 This position reports to the Manager Community Living Center (CLC) Duties will be performed primarily in the CLC Alpha; however, reassignment may be required based on resident care needs. Major Duties include but not limited to: Communicate respectfully and work effectively with staff, health care team, residents, and visitors. Use judgement in selecting the most appropriate application of procedures and guidelines, varying the order and sequence of assigned procedures based on resident's condition. Prepare and administer prescribed oral, topical, subcutaneous, inhaled, injectable medications, and suppositories according to policy and procedure. Correctly documents medications given or omitted in Bar Code Medication in accordance with established policy and procedures. Initiates, maintains, and discontinues IV therapy and intermittent intravenous devises. Administers IV piggyback medications via continuous infusion or intermittent infusion devise according to medical center policy and procedures. Report all untoward resident incidents for example: medication errors, falls, etc., to Charge Nurse/Nursing Manager, and initiates incident report. Observe and document resident changes in condition, such as bleeding, vomiting, fainting, skin changes, behavioral concerns, etc. and report changes to the Charge Nurse or Registered Nurse (RN). Initiate emergency response teams: Rapid Response Team (RRT), Code Blue, Stroke-Cardiac alerts and responds to emergency situations including cardiopulmonary arrest, choking, and disaster or fire drills. As an interdisciplinary team member, provides input into the care planning process, assists/provides teaching in activities of daily living (ADLs) to residents. Provides/reinforces resident/family/significant other teaching related to medications, disease process, procedures, and self-care activities. Participates in more complex teaching for resident self-care, such as diabetic care, self-catheterization, etc. and in restorative program based on the needs of resident. Perform treatments/procedures utilizing established nursing care plans and auxiliary equipment according to policy, including but not limited to: application of heat and cold, wound care, changing sterile and clean dressings, administering suppositories, cleansing and retention enemas, tube feedings, weights, vital signs, glucose finger sticks, tracheostomy care, specimen collection, intake and output, urinary catheterization, oxygen, and colostomy irrigation, orthopedic traction, spirometry, nebulizer treatments, and standard 12 lead EKGs, and care of well-established tracheostomies and laryngectomies. Performs oropharyngeal, tracheostomy, and tracheal suctioning, and tracheostomy and laryngectomy care. Perform resident care activities which include, but are not limited to: Restorative Care, oral and enteral feeding, bathing, bed making, lifting, turning, positioning and ambulating residents; oral, hair, nail and related care; and other related care such as care of urinary drainage appliances. Collect specimens such as urine, sputum, and stool for laboratory analysis. Provide range of motion exercises for dependent residents at least once per shift. Adhere to the Safe Patient Handling Program standards, practices, and recommendations. Prepares residents for tests, examinations, treatments and surgery. Assists Licensed Independent Practitioner with a variety of diagnostic procedures by setting up trays, supplies, and equipment, using sterile technique and monitoring the resident's condition before, during and following the procedure. Procedures include, but are not limited to: insertion of chest tubes, thoracentesis, paracentesis, lumbar puncture, and liver biopsies. Applies assistive/protective devices or prosthetic appliances. Understand restraint policy, has knowledge to apply, observe and care for residents in restraints, documenting according to hospital policy and procedure. Document observations, therapeutic measures performed, and care provided following established policy and procedure in correct medical record and completes clinical reminders. Collect subjective and objective data which contributes to the nursing admission assessment, resident reassessment, and discharge planning.
