Choice Healthcare Services is a Pediatric Dental Service Organization with over 40 offices in California. We are looking for providers to work 2-10 days a month in various offices.

Requirements:

• Own Mobile Unit
• Provide all medications
• Willing to be enrolled in Denti-Cal
• Pediatric Endorsement to treat children under 7 yrs old
• Dental Anesthesia license in CA or Dental GA permit in CA
• Able to provide services in our dental clinics

• Antioch (1-2 days/mo)
• Chico (16 days/mo)
• Fresno (2-4 days/mo)
• Lodi (10 days/mo)
• San Francisco (12 days/mo)
• Modesto (5 days/mo)
• Natomas (11-14 days/mo)
• Vallejo (5 days/mo)
• Vacaville (10 days/mo)
• Yuba City (4-7 days/mo)
• Reno (2-4 days/mo)

• Daily minimum guarantee - $3500 (negotiable)
• Pay per case - $600-650/60min; $700-750/90min

• Avg 6-8 cases/day
• 1099 contractors only
• Patient base if 85% denti-cal, but we accept ALL insurances
• Typical day is from 7a-3p
