Department: Home Care Services

Position Description: Home Care Aide

Status:  Part-Time No Benefits: 23 or less hours per week. 

Shift: Day - 8:00 a.m. to 4:30 p.m.

Days: Monday thru Friday; including 1 weekend per month

Position Summary: Provides personal care and supportive services to home care clients in their place of residence to promote rehabilitation, safety, and independence within the individual's physical and mental limitations.
Education/Qualifications: High School Diploma or equivalent; Must have 75 Hour CNA training and certification.  Requires current driver's license and auto insurance, with the ability to travel throughout Boone County.
Essential Functions:
  • Regular attendance.
  • Excellent verbal and written communication skills
  • Possess or have access to reliable transportation for all of Iowa's seasons
  • Ability to think and act independently when required
  • Confidence in providing personal cares and assisting with ADL's
  • Knowledge of computer and mobile devices for training and documentation purposes
  • Excellent organization and time management skills
  • Consistency in keeping on schedule and attending appointments on time according to home care's policies and procedures.
  • Demonstrates knowledge and skills necessary to provide care appropriate to the ages of the patients served within the department.
  • Maintains performance in compliance with State and Federal laws, and consistent with high standards of business and professional ethics.
  • Provides personal care assistance to patients such as bathing, grooming, dressing, simple wound cares, ambulating, and rehabilitation.
  • Assists patients with oral medications as directed by home care professional.
  • Provides meal planning and preparation as directed by supervisory staff.
  • Provides basic light housekeeping such as sweeping, mopping, vacuuming, dusting, cleaning bathroom/kitchen areas, changing linens, and laundry.
  • Provides miscellaneous duties such as grocery shopping, errands, money management/bill paying, and parenting skills.
  • Documents services provided according to documentation standards of the department.
  • Reports changes in patient condition to supervisor.
  • Maintains effective communication with all levels of supervision and staff.
  • Maintains HCA scheduling planner, and attends regular scheduling meetings.
  • Attends and participates in department staff meetings/in-services and hospital wide mandatory in-services.

WORKING CONDITIONS:

Hours may vary depending on patient census. The employee is regularly exposed to outside weather conditions including driving. The employee is also exposed to varied home environments including pets and household pests, and cluttered conditions. The employee is occasionally exposed to wet and or humid conditions, moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electric shock. The noise level in this work environment is usually moderate, but can vary with each home situation.

Vision must be correctable to view computer screens and read printed information. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision and ability to adjust focus.
