As a member of the Wentworth Health Partners Cardiovascular Group and the Heart Failure Program, the Nurse Practitioner will assist with the development, implementation, and evaluation of a program of specialized health care for a patient population with heart failure and other cardiac disease. This position will report to the Practice Manager of the Cardiovascular Group for practice operations . This is a full time position which will require flexibility to meet the needs of the high-risk patients. The Nurse Practitioner will promote a collaborative relationship among various health care professionals toward continuity of patient care and efficient use of resources. The Nurse Practitioner will serve as a role model for professional nursing practice. The appropriate candidate will have previous inpatient cardiology experience preferably in the area of Heart Failure and coronary artery disease. It is anticipated that the majority of practice will be devoted to transitional management of high-risk cardiology patients. The Nurse Practitioner may provide support in the inpatient and outpatient clinic settings to ensure continuity of patient care.
Graduation from accredited Nurse Practitioner Program or PA Program Master's Degree required NH License Preference given to candidates with previous experience in a heart failure clinic or inpatient and outpatient cardiology.
