Provides advocacy, support, resource navigation and ongoing case management for patients seeking care for a substance use disorder.
Experience Minimum Required
• 1-2 years providing care coordination in a healthcare related field, including experience in the field of substance use
• 3-5 years providing recovery support and care coordination. Experience working with pregnant women; HIV/AIDS; veterans; and/or
Education Minimum Required
• Bachelor's degree in a healthcare related field
