Be a part of a world-class academic healthcare system at UChicago Medicine as a Patient Advocate for our Community Health division at our main medical campus. Note: This is a grant funded role. The Patient Advocate role supports the University of Chicago readmission efforts by working closely with patients, UCM staff and community partners to educate and obtain post medical care for patients.
In this role, the Patient Advocate will identify patients who lack a regular source of primary care and link them to a medical home in the community.
Essential Job Functions:
- Form a relationship with patients who would like linkage and referral services.
- Follow up with patients to determine success of linkage and to offer further problem-solving assistance as needed.
- Help patients identify a community resource for follow up care that matches their clinical needs and personal priorities.
- Assist patients to make a follow up appointment.
- Follow up with participating agencies as needed to secure patient access.
- Act as an advocate, when needed, on behalf of patients with service providers and with outside agencies.
- Document all client contacts, follow-up, and referrals.
- Document patient responses to collaborative methods and approaches.
- Participate in all staff meetings, in-services, trainings, and committees as appropriate.
- Works closely with other patient advocates and social workers in the ER to ensure strong communication about patients who do not have a medical home.
- Assist in the development of system-wide methods for linkage and referral.
- Performs other duties as assigned.
Required Qualifications:
- A high school diploma or GED equivalent
- Experience in customer relations or relationship management
- Outstanding interpersonal skills
- Familiarity & interest in the surrounding community
Preferred Qualifications:
- Experience working in a healthcare setting

- Shift: 8-hour shift – 8:00am to 5:00pm – No weekend coverage
