POSITION SUMMARY:
The Home Health Licensed Practical Nurse (LPN) will provide skilled nursing care to patients in accordance with physician's orders, directed by the RN, and in compliance with Agency policies/procedures, State and Federal regulations.
ESSENTIAL FUNCTIONS: To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including but not limited to:
- Provide nursing care and prescribed treatments for patients in accordance with physician's orders and in compliance with company policies/procedures, state and federal regulations
- Implement the patient's individualized plan of care, under the direction of the Registered Nurse, in compliance with company policies/procedures, state and federal regulations
- Participate in on-call rotation
- Willing to travel to other office locations to assist with staffing as needed
- Maintain appropriate documentation of care provided to patient and any communication with the patients' physician and patient representative(s)
- May provide personal, and hygienic care for patients
Required Skills:
- Work requires advance knowledge of the Homecare Industry, Medicare Regulations, CMS, and other State/Federal regulatory changes
Required Work Experience:
- One (1) year experience as a Licensed Practical Nurse preferred
- One (1) year home health experience preferred
Licensing/Certification/Education Requirements:
- Graduate of an accredited Practical Nursing (LPN) Program
- Current License Practical Nurse (LPN) license
- Current CPR Certification
Other Requirements:
- Position may require unscheduled overtime, week-end work
- Must be willing and able to work flexible hours
- Travel required
Physical Requirements:
- Job requires frequent lifting of objects up to 40 pounds
