Description
Providence TrinityCare Hospice is seeking a full-time Palliative Care nurse practitioner to join a comprehensive, multidisciplinary team. This position will be a fairly autonomous role for a small Palliative Care program, which is part of a larger regional program. TrinityCare is known in the community for its outstanding accomplishments and company values. Conduct meaningful care and enjoy working in the San Fernando Valley, a beautiful community within Los Angeles County.
- Monday to Friday schedule; 8-hour shift
- Outpatient care only
- Telehealth position
- CA license required, local candidates preferred
- 1 to 2 years of experience is highly preferred

Providence TrinityCare Hospice is one of the oldest hospice programs in the greater Los Angeles area, with a staff of 180 professionals who provide compassionate end-of-life care to more than 30,000 patients and families. It has received numerous commendations from the Community Health Accreditation Program for its innovative programs, excellence in community health care practice and quality performance. The facility consults and partners with all six Providence medical centers in Southern California.
