The Valley Health System has expanded into an integrated health network that serves more than two million people in Southern Nevada. Starting with Valley Hospital Medical Center in 1979, the Valley Health System has grown to include Desert Springs Hospital Medical Center, Centennial Hills Hospital Medical Center, Spring Valley Hospital Medical Center, Summerlin Hospital Medical Center and our newest addition Henderson Hospital.
The Director of Case Management is responsible for the assessment, planning, coordination, development and evaluation of the Case Management program. This includes the functions of case management, utilization management, discharge planning and social services. The Director will be accountable for the assessment and monitoring of the care and services rendered by the department staff. Also responsible for staffing levels, competency and educational needs of the case management staff to ensure effectiveness of the coordination and control of resources. Provides consultation and education to facility leadership, clinical and financial personnel and medical staff members related to case management, utilization and discharge planning practices. Ensures compliance with local, state, federal and private agencies as relates to Case Management and Utilization Management. Provides leadership in the fulfillment of UHS/VHS driven initiatives and expectations.
Graduate of an accredited school of nursing.
A minimum of two years experience in leadership/management position. Candidate must have knowledge of social and physical factors that affect functional status at discharge, and knowledge of community resources to meet post discharge clinical and social needs.
Has a current license to practice as a Registered Nurse in the State of Nevada.
Must be able to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patient.
