Job Definition SCOPE OF POSITION
Community Health Workers are individuals who come from the communities they serve. These Workers shall have the skills and experience to understand what low-income patients are going through and help them get through difficult times. The Worker will act as caring neighbors to help potentially eligible individuals, or families, to assess eligibility for state-sponsored health coverage programs and to assist those individuals and their families with the application and enrollment process for those programs.

EDUCATION AND EXPERIENCE
Bachelor's degree required. In lieu of degree, equivalent five years working in a community resource or social work environment. Completion of a Ballad Health provided Community Health Worker Training program upon hire. Long-time resident of the community to which assigned with good knowledge of resources in this community.
Valid and active CHW certification preferred
